Cementing his status, he defended the IBF title five times with wins over Junior Witter, Terron Millett, Hector Quiroz, Reggie Green and Allen Wester.

Judah defeated Rafael Pineda by split decision in 2004 and won a rematch against Spinks in 2005 to become the new undisputed welterweight champion.
The following year, Judah defeated Cosme Rivera in his first fight as undisputed champion by knockout in the third round in 2005.
The rivalry between Cosme and Andre Berto continued, with Cosme defeating the much more powerful Berto in an unexpected victory in 2007.

In January 2017, Judah returned after a hiatus to face Jorge Luis Munguia, winning via technical knockout in the second round.
A year later Judah fought Noel Mejia Rincon in Calgary, winning by unanimous decision over 10 rounds despite suffering an injury in the 6th round.
On June 7, 2019, the 41-year-old Judah was stopped in the 11th round by Cletus Seldin, which forced him to be hospitalized with a brain hemorrhage that likely ended his career.
